Your First Visit
What to expect:
Your first visit is what we refer to as the “initial evaluation”. The initial evaluation will take roughly 60 minutes. During this time, the Physical Therapist will do an assessment of your injury/injuries. This is the time to discuss any questions concerns, or even goals you may have in relation to your injury/injuries.
Please bring with you:
- Drivers License or a form of identification
- Insurance Card(s)
- Doctors Referral (if you have one)
- Co-pay (if needed)
- Health information relating to your visit (Doctor’s notes, prescriptions, MRI/X-Ray reports etc.)
What to Wear:
Please wear comfortable clothing that allows for easy access to the body part relating to your injury.
